Pilot/ATC Employee Exam Prep Checklist

 

Checklist for your visit:

1. Complete the FAA Form 8500 online

2. Double check your form for accuracy

3. Bring your special issuance letter, if applicable

4. Obtain any medical reports or pertinent official documents

5. Bring all labs or imaging reports that may be relevant

6. If applicable, complete an ophthalmologic Exam FAA Form 8500-7

7. Bring three recent blood pressure measurements, if applicable

8. Call with any questions
